It can mean any one of a number of things: 1) subscriber mailbox did/does not verify at the time of processing 2) a secondary MX host (not normally used) WAS used and is misconfigured 3) misconfigured primary mail server Send the subscriber a copy of the report and see if he bounces again, noting the RECEIVED line headers.
