Q1: how do they know it was distributed (unless they have another subscription or see it in the archives)?

Those I'm speaking of are trusting. ;-)     They could have "Ack", which comes pretty close to knowing it was posted.   Besides,  almost all of the posters have Google Apps mail, so they don't see their own post anyway!

Q2: owners can post w/o a subscription -- how are they (self-) moderated with the options show in your OP? 

"Confirm" doesn't confirm for an owner?   Almost all are not an owner for the scenarios I was describing.
 

Q3: are there any generic posting addresses e.g., "roll" accounts?

I don't know the term "roll: account.  Some people have a functional account they use for various purposes, including posting to lists and replying to client questions.
