What kind of settings cause headers to display this way (double headers, no
separator lines between messages)?  This started happening all of a sudden.

Here are my settings:

Subscription options for John Witherspoon <[log in to unmask]>, list
SHOTIMES-L:

DIGEST         You receive list digests, rather than individual postings
MIME           You prefer to receive messages in MIME format
FULLHDR        Full (normal) mail headers
NOREPRO        You do not receive a copy of your own postings
ACK            Short e-mail acknowledgement of successfully processed
               postings
