On one list that I co-operate, the MAILTPL file contains (among other
templates) these two lines:

>>> POST_EDITOR Message sent to user when posting is forwarded to editor
.qq

This stops the generation of messages to REVIEWed subscribers.  This
won't solve your problem but might ameliorate it esp. for (entirely) new
subscribers.

In terms of pathological Whack-A-Mole(*) subscribers, you would be hard
pressed to know in advance what ISP and userid they are coming from.
