Thank you for the insight and the information.  I "reverted" to the default v16.0 template and the problem was solved.

Yes.  This is handled differetnly in 16.0.  However, if you have previously
edited the OBJECT-LOG-DATERANGE template in 15.0 or 15.5, your changes now
override the new default in 16.0.  You can continue to annually and manually
edit the OBJECT-LOG-DATERANGE template, adding new years as necessary, but
this is not recommended.

Instead, go to this template as if you intended to edit it and then click on
the 'Revert' button at the bottom.  This will remove your edited version and
allow the new 16.0 default template to be used.  This is the preferred method.
