I have seen this with [log in to unmask] (1.8d).  I asked the
LISTSERV admin about it, but he did not suspect anything wrong other than
user error.  I don't have as much evidence as you do to prove it was not
user error, though.

Since then, my list has moved to passive probes, and this problem has gone
away.  Not fixed, mind you, but at least it doesn't bite us anymore.
