That your list is moderated has no effect on how subscriptions are
handled, the former is governed by the Send=  keyword and the later by
the Subscription=  keyword, so yes, you can have both.
 
GET your list header, edit it so it reads Subscription= Open,Confirm  ,
then PUT it and you're set.
