Is the "held" message, the one sent to those who post to the list when
the listowner has placed the list on "hold," in the default.mailtpl?
If it is, where is it, I can't find it.

I think there are two, one for an automatic hold, when the list exceeds
its threshold, and another for when a listowner intentionally places the
list on hold.

Are these in the default.mailtpl or are they "hard wired"?
