Typically, this may result from a subscriber forwarding to another
account, or an "exploder" (remailer) which doesn't recapture its errors
correctly.
 
One possible clue in a couple of the headers you quoted:

Would you happen to have a user "[log in to unmask]" in your list?
Seems likely that might be a user forwarding to "[log in to unmask]".
