I missed your post so this answer may be a bit late.  You can look at our
online interface: http://listserv.towersoft.com.au/cgi-bin/WA.EXE

I managed to easily insert links to both the Password Creation and the
Cookie Reset pages.  I forget exactly which template I had to modify but I
remember it was the only one that dealt with the main entry page.
