I have confirm set on ALL of our open lists here, 100% of the time, when
I get the same complaint from a user, I ask them to redo the procedure
and cc: me.  The headers invariably show that the user sent the ok+code
via a REPLY instead of a SEND as per the instructions - when I point this
out, the problem goes away  ;-)
