Paul:  I would put a disclaimer at the top of every posting (use the Top Banner) reminding posters that it is their responsibilty to protect patient confidentiality in any postings to the list.  Seems like you'd also want a statement about the responsibility to exercise their own clinical judgment in making patient treatment decisions.

If you have a Risk Management/legal department, I'd have them craft the wording for you (lawyers are good for a few things...namely speaking legalese....)
