The occasional 30 min. lag time is between the time I receive the approval acknowledgement and the time LSMTP actually begins processing the approved message out to the subscribers. I use a workstation to approve messages rather that sitting at the server, but the server is in the same room. I can see (and hear) the server activity easily, as well as see the results in my own e-mail (I subscribe to all the lists I moderate).
