We had the same experience, and I wasn't made aware of it until it had
been going on a week or so.   All you can do, I think, is resend the
tailored form to your postmaster/listserv manager and ask her/him to
replace the default mailform.  If there is some other way, I'd like to
hear about it.  Our manager hasn't gotten around to it yet, despite
repeated pleas.
