The list that looks like a numbered list of messages at the front
of a digest isn't.  It's a numbered list of subjects.  If there
are multiple messages in the digest on a particular subject there
will be only one entry in the list at the front of the digest.
You will then find a parenthesized number at the end of the subject
line in the list which gives the number of messages in the digest
on that subject.  Ergo the list is a table of subjects not a
table of contents.
 
I haven't checked but I wouldn't be surprised if RFC1153 says
that's the way it's supposed to be.
