We were hoping to create a form to collect the information (email
     address and name) and send it from there.  Relying on users to use
     their browser for mail won't work.  (Unfortunately!)
 
     Hopefully someone else has another setup?

I have a working subscription link on one of my websites.  I simply
used the mailto function.  You might include something like the
following in your html file:
 
To subscribe, please click <A href="mailto:listserv@yourlistaddress">
here</a> and type SUBSCRIBE LISTNAME YOURFIRSTNAME YOURLASTNAME on
the first line of the blank message screen.  If possible, turn off
the automatic signature function (if you use it) in your mail
utility.
 
This will only work if people use the same account/address for e-mail
and web-browsing.  I also included instructions on how to use our
"confirm" option.  This option has been working well.
