I am looking to setup an online subscription form for a couple of related
lists. I would like the end result to look like the following:
http://www.digitalriver.com/insider/list/testing/sample1.html

Essentially the goal is to have requests processed for more than one list at
a time.

I am also interested in seeing some other, more creative layouts than the
default, if anyone cares to forward along some links.
