Note also that the spaces are there to help avoid confusion.  Certainly
we could write
 
Send= access-level[,Semi-Moderated|Hold|Confirm|Hold,Confirm]
 
but I'll bet a lot of people would look at that and go "Huh?"  In any
case the examples never show the spaces.
