Both work.  The "|" symbol is confusing, I grant you, so I'll make it
less confusing in the next edition.  Technically it should probably be
something like
 
Send= access-level [,Semi-Moderated | Hold | Confirm | Hold,Confirm]
 
Does that make more sense?  :)
