Broken sendmail rewrite rule...generally found in pre-8.7.x sendmail.
You've got the mail routed through an 8.6 sendmail, if you upgrade it to
8.7.x or later this will likely go away, as the default 8.7.x sendmail.cf
does not appear to inherit this bug from 8.6.x.

And no, I don't know which ruleset in sendmail.cf is causing the problem :)
