Most likely, the address they are entering through the web interface does
not agree with the return address their mailer sends back when they try to
confirm.  (E.g., if you enter "[log in to unmask]" but your mailser shows
"[log in to unmask]" in your outbound mail.
 
The solution, of course, is for the potential subscribers to be more aware
of what their mailers put in their outbound mail and/or more careful to
match it.
