I'm hesitant to say this, but . . .

Most old fashioned DOS wordprocessors would allow you to write macros 
to  convert _any_ list which offered that information to that format. 
I use WordStar 7 and WordPerfect 6.2 for DOS, and it takes about a 
minute to write the macro and no time flat to run it . . . there may 
be a way to get GUI  based wordprocessors to do it, but I've not 
discovered them (Bill Gates and Steve Jobs want to you to point & 
click repetitively: they must once have had stock in an RSI clinic).
