All excellent points. I might just add:
On PAGEMAKR several dialup users outside the U.S. have written to let
me know their provider charges them *by the byte* for downloads, which
can add up fast since spams are generally quite lengthy. (I received one
recently with a CC: line that had over 1900 addresses in it!) Come to
think of it, AOL subscribers would have the same complaint...
