web php script to subscription

I have confidential (Confidential=yes) wich requiere password (Log in) to suscribe to them. But I'd like that a person who want to suscribe no need Log-in. The only solution is to create a web form for suscription to list.

Does anyone have any web php script (code) with procedures for Subscription to a  list?
