In the "it shouldn't happen" category, is it possible that the browser is
sending the request a first time, the server gets it, and sends a TCP ACK,
which is then lost, and the retransmit is treated as *another* request?

Yes, it *should* be discarded based on the TCP sequence numbers - but I've seen
broken TCP stacks before.

What operating system/webserver are the people seeing the problem using?
