It's a browser problem, or perhaps a network problem on your end.
(We have occasionally had a few people with similar problems, even in 1.8d.)

Here are the actual log entries.  They show that your first click succeeded in
the confirmation, however, before the results of that operation could be
displayed back to you, several more clicks arrived at our server, causing the
'no pending command' response, which is by then an accurate response since the
first click already did the approval.  In this case your e-mailed OK was
invalid, so the first 'click' was the first and only actual confirmation of
your subscription.
