Most likely the list is in 'HOLD' status either because of exceeding the daily
quota or (more likely) a typo in the directory path for the list message
archives or perhaps insufficient permissions for LISTSERV to write to that
directory.  If you send a message to the list and then immediately look in the
LISTSERV log file, this will be obvious.  It may also be that the list is
moderated and that messages require Editor/Moderator approval.  Check the
moderation queue for unapproved messages.  Again, this will show in the
LISTSERV log file.

No.  They are index files used for searching the message archives.  They have
no effect on sending.

The files will not be created until a search is performed on the list message
archives.  Obviously there also needs to be list message archives to index. If
no messages have been sent to the list then there will be no message archives
to search/index.
