I've got a moderated list on my server that has the header below.

The owner/moderator has complained that she is seeing some notes on the
list which she has not confirmed. When I look at some of the sample notes
I do see "Approved-by:" header lines that show an email address other than
the moderator. They match the sender of the note. Most notes have an
"Approved-by:" that correctly matches the moderator id. Part of one of
the odd note headers follows below.

The list header looks ok to me for a moderated list, but there is a lot I
don't know about list headers. Can anyone offer some pointers here? We
are running LISTSERV 1.8d at 2000a maintenance level on AIX.
