Yesterday we experienced this problem. The list owner's alias pointed to another address and for some reason the mail never went out to the subscribers. We deleted all aliases to the list owner valid list address and then the mail posted to the list went out successfully. 
One note, you have 'send = editor, hold, confirm' you should be getting a confirmation (1x) to post the message to the list. Is this occurring?
I wonder if adding 'mail-via=distribute' to the header would help.

I'm still having problems with one of my lists.  The issue is as follows:

I am an owner and editor.  I post a message to the list.  I get an
informational mail that says my message was distributed to all the members
of this list (currently number 1020).  No one receives the message.  I look
in the LISTSERV log and see lines indicating my message was received,
processed, and then delivered -- to 0 recipients.

Any help you might provide is very much appreciated.            CF
