Your understanding is incorrect
Send= Editor,Hold,Confirm
means that anyone in the world (unless you had SERVICE=) will have their
postings submitted for moderation.
Ben Parker of L-Soft had mentioned a particular configuration:

Actually, we had (and have)
DEFAULT-OPTIONS= NOPOST
with authorized posters SET either REVIEW (Ben recommended against that)
or POST.
