Create a file called HORROR MAILFORM.
Copy the sections :ADD1 and :SIGON from the file $DEFAULT MAILFORM on
Listserv's A disk.   Change the text in both the :ADD1 and :SIGNUP1 sections
between the lines :ADD1 and .im SIGNUP, and :SIGNUP1 and .im SIGNUP
or you may leave out the .im SIGNUP.
 
If you want portions of the text to not be reformatted by Listserv, add
.FO OFF before the text.
 
When ready, the file is to be placed on Listserv's A disk by the Listserv
postmaster.
