RFC 1047 describes a problem inherent to SMTP which may have been
the cause of the multiple deliveries.  Depending on what is being
logged, you may not see each delivery attempt, but only the successful
one.

I see two possibilities:

  (a) the RFC 1047 problem happened between LSMTP and the next hops, or
  (b) the multiple deliveries were caused by local problems at the
      destination end.

If (a) was the case, then you can try to avoid the problem by raising
the SMTP timeout interval (in LSMTP's SMTP settings).  If the problem
repeats itself then maybe you should enable logging of outgoing
connections (by creating a site definition).  Then you can see exactly
what is going on.
