Checked both listserv.log and sitecfg.file.  The observed delay does not match the value of the SPAM_DELAY site configuration variable.

Check your LISTSERV log for mention of spam delay at the time the incoming item arrived, for which the approval request is delayed. There is a chance the original message looked like spam, perhaps.  Does the observed delay match the value of the SPAM_DELAY site configuration variable?
